To request a printed copy of the provider directory or for more information on a particular provider, call (800) 841-2790, submit a request electronically or in writing to America’s Best Vision Plan (ABVP), 1202 Monte Vista Ave #17, Upland, CA 91786, and ABVP will send a copy postmarked no later than five business days following the date of request.
To report directory inaccuracies, please contact the plan by calling (800) 841-2790, sending an email to [email protected] or visiting our website.
ABVP Plan Members are entitled to full and equal access to covered services, including enrollees with disabilities, as required under the Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act of 1973. You are also entitled to obtain an interpreter at no cost to talk to your doctor and/or health plan. To get an interpreter or to ask about written information (in your language), first call America’s Best Vision Plan at (800) 841-2790. You may also call the HMO Help Center at (888) 466-2219.
