Tip #1: Brush Up on Your Health History
Before you arrive, you’ll be asked to fill out a brief online medical questionnaire.
In the days leading up to your exam, ask yourself:
- Have I noticed any eye problems, like blurry or double vision?
- Has my vision kept me from doing any activities?
- Have I had a recent illness, injury, or surgery that my eye doctor should know about?
- Have I started a new medication?

Before you leave home, be sure you have the following:
- Your current eyeglasses and/or contact lenses

- The name and contact information of your primary care physician
